Digital Headache Diary pilot for Migraine Care at Spire Hesslewood Clinic

Thank you for your interest in joining our Digital Headache Management Pilot. Your participation is an important part of shaping how we care for people living with migraine—making it more efficient, personalised, and empowering.

This pilot project, in collaboration with our digital health partner Migrevention, is focused on improving how migraine symptoms are tracked and managed using a digital headache diary, instead of a traditional paper diary.

The digital diary, a mobile app, is designed to be simple and easy to use. You will receive clear instructions, and support is available should you need help getting started.

Your involvement is voluntary, and you can stop using the diary at any time. However, by taking part, you are playing an essential role in testing a more modern, patient-centred approach to migraine care—one that has the potential to benefit many others in the future.

What will your journey be like?

1. Attend your clinic appointment

  • Attend your scheduled appointment at Spire Clinic

  • Your Neurologist will assess your headaches

  • You will be invited to join the pilot and scheduled (a) remote nurse consultation after 2 weeks; (b) in-person nurse consultation after 3 months

2. Download the diary and track your headaches

3. Connect to the digital clinic

  • During the first few weeks you will receive an email invitation from the clinic

  • Click the connection link in the email

  • Complete the consent form in the app

  • Confirm the connection to the clinic dashboard

  • Your clinical team can now securely review your reports.

4. In a few weeks: nurse consultation (phone)

  • Attend your scheduled 2-week telephone consultation

  • The nurse reviews your diary report with you

  • Agree the monitoring period and next appointments

5. Continue tracking and attend 3 month follow-up

  • Continue recording your headaches

  • Attend your scheduled in-person nurse consultation

  • Your report will be reviewed again

6. Complete feedback and next steps

  • You will receive a short feedback survey by email

  • Complete the survey as part of the pilot evaluation

  • If further monitoring is needed, you will continue tracking

  • If not, your participation in the pilot is complete

  • Migraines affect millions of people, yet traditional approaches to managing them—like paper diaries—can be difficult for patients to maintain and challenging for clinicians to interpret. This pilot, run by Dr Fayyaz Ahmed in collaboration with digital health partner Migrevention, explores how a simple mobile app can make migraine care more efficient, personalised, and easier to manage.

    By testing a digital headache diary, we aim to improve symptom tracking, reduce waiting times, support better communication between patients and clinicians, and help prevent issues like Medication Overuse Headache. Most importantly, we're working to find out whether this modern, patient-centred approach can improve quality of life for people living with migraine.

  • Migrevention’s Digital Headache Clinic is an innovative, patient-centred approach to supporting the management of primary headache disorders. As part of this Spire Clinic pilot project, the focus is on the use of the Migrevention Headache App as a digital clinical diary integrated into the clinic pathway to support headache assessment, structured monitoring, and specialist care. Within the pilot, the pathway includes onboarding by the GP or Neurologist, digital tracking by the patient, review by the Nurse, and coordinated oversight by the Patient Manager.

    Headache App for Patients

    The Migrevention Headache App is a user-friendly mobile application that functions as a digital headache diary. It enables patients to systematically record headache episodes, medication use and effectiveness, potential triggers, and relieving factors as part of their routine self-management.

    Following agreement to participate in the pilot, patients download the app and complete registration. They then receive an email invitation from the clinic to connect their diary to the clinic dashboard. By following the secure link in the email, the app opens and the patient completes a consent form within the app to enable data sharing. Once confirmed, the clinical team, including the Nurse and Patient Manager, can securely review structured headache reports via the clinic dashboard.

    The app is certified as a Class I Medical Device in the EU and is designed to support structured data collection for clinical use. By tracking headaches consistently over time, patients gain clearer insights into their condition, while specialists receive objective, longitudinal information to support clinical assessment and decision-making.

    Within this pilot, headache reports are reviewed by the Nurse during a scheduled 2-week telephone consultation and again at follow-up. The Patient Manager reviews reports within the dashboard, supports coordination of care, and oversees the monitoring pathway. A patient experience survey (PREM) is sent by email as part of the pilot evaluation. Where further monitoring is required, patients continue tracking; otherwise, participation in the pilot concludes.

    Supporting Care Through Digital Monitoring

    By integrating the Migrevention Headache App into the Spire care pathway, this pilot explores how structured digital headache tracking, combined with coordinated review by the Nurse and Patient Manager, can enhance communication between patients and specialists, improve understanding of headache patterns, and support evidence-based management of primary headache disorders within routine clinical care.

As part of the pathway, your diary can be securely connected to the clinic dashboard following an email invitation and in-app consent, allowing your care team to review your reports as part of your scheduled consultations. This can help you better understand your condition and feel more in control of your care.

    The app makes it easier to share accurate, structured information with your clinician, as your reports are reviewed by the Nurse and overseen by the Neurologist within the clinic pathway. This supports more personalised advice and timely, effective treatment decisions during both telephone and in-person follow-ups. It may also help reduce the risk of Medication Overuse Headache by giving you clearer insights into your medication patterns over time.

    Most importantly, this digitally supported monitoring approach is designed to improve communication between you and your care team, enhance the quality of your consultations, and improve your overall care experience and quality of life.
